Get Appointed with Pacific Life

Are you a registered financial representative who would like to offer fixed and variable annuities from Pacific Life?

If so, you will need to get "appointed" with us.

Check with Your Broker/Dealer

To sell Pacific Life products, your broker/dealer must have a fully executed selling agreement with us. If they don't, ask them to contact Pacific Life at (800) 722-2333 or in New York at (800) 748-6907. A customer service representative will be happy to initiate the selling-agreement process.

Complete a Pacific Life Appointment Data Sheet

Once your broker / dealer has a selling agreement with Pacific Life, fill out an Appointment Data Sheet. Mail or fax it to us along with a copy of:

  • Your state insurance license(s) for each state in which you will do business.
  • Your FINRA Central Registrations Depository (CRD) Status Report that reflects your current Broker/Dealer.
  • State appointment forms (if applicable).
